Acupuncture has the potential to depolarize C and Aδ fibres, and via the anterolateral tract, activates the periaqueductal gray matter (PAG) in the brainstem, which, through a long loop pathway, promotes an analgesic effect at the spinal segmental level of the needling area, through the release of serotonin and adrenaline in the dorsal horn of the spinal cord. This is called the descending pain inhibitory pathway, one of the main analgesic pathways in acupuncture and other segmental therapies.


Research has consistently shown that acupuncture stimulates the release of the body's own natural painkillers (endorphins, enkephalins, and dynorphins), which bind to the same receptors targeted by opioid medications, but without addiction or side effects.
At the site of needle insertion, acupuncture triggers a mild, controlled local tissue response. This stimulates the release of vasodilatory neuropeptides such as substance P and calcitonin gene-related peptide (CGRP), which increase local blood flow and promote tissue repair.

Brain Neuroplasticity and Central Sensitization
In chronic pain states, the nervous system often undergoes "central sensitization", where the brain becomes hypersensitive and amplifies pain signals even in the absence of ongoing tissue damage.
